Once your child's One Page Profile is developed, a 'Provision Map' will be put in place to show which interventions we will use to support your child. These Provision Maps will be updated weekly, based on progress, and their effectiveness evaluated every half termly the class teacher and the SENCo to ensure successful outcomes and to plan for the following year. Your child's One Page Profile will also be taken into account when planning for the whole class to ensure that your child's needs are met within the wider classroom. If several children have similar needs, group targets or strategies can be used as part of focused classroom practice.
Sometimes a child may need support from outside agencies, for example Speech and Language support. In this case the SENCo and parent/carer complete a referral form. If a programme of support is recommended, the SENCo ensures that it is followed.
